Climacteric:- is the phase of waning ovarian activity and may start two or three years before the menopause and continue for two to five years after it . The climacteric is thus a phase of adjustment between active and inactive ovarian function and may occupy several years of women’s life. Menopause occurs between the ages of 45 and 55 years the average age being 47 years. Menopause before the age of 40 is called premature menopause. Late menopause is also common in women suffering from uterine fibroids and in the prone to endometrial carcinoma. Phisiology :- During climacteric , ovarian activity declines. Thus the menstrual cycles tend to become an ovulatory and irregular Harmonal levels:- Once menopause is well established, plasma oestrogen level may remain low at 10 – 20 pg/ml. Oestrone level varies between 30 and 70 pg/ml. The ovary also secretes a small amount of testosterone -responsible for mild hirsuitism noticed after menopause. The genodotrophin harmone appears in high concentration at menopause. Menopausal Symptoms:- In majority , apart from cessation of menstruation, no more symptom is evident. 1)Menstrual:- The three classical ways in which the period ceases are a)Sudden cessation. b)Gradual diminution in the amount of blood loss with each regular period until menstruation stops . c)Gradual increase in the spacing of the periods until they cease for atleast an interval of 6 months. 2)Cardiovascular :- The commonest symptoms are hot flushes and sweating which are the hall mark of the climacteric in 85 % of women. The functional derangements of cardiac action causing palpitations are common and angina pains of effort are complined of at this time. 3)Neurological and Emotional :- Paraesthesia , which takes the form of sensations of pins and needles in the extremities is common .Headaches and noises in the ears of complained of ,while physical disturbances which the form of irritability , depression and even melancholia are frequent . The condition of pseudocyesis is infrequent. 4) Locomotor system:-Menopausal arthropathy , osteoarthritis , fibrositis , back ache,and vertebral disc lesions are all common complaints at menopause. 5)Osteoporosis :- Bone resorption follows oestrogen deficiency after menopause. 6)Urinary tract:- Menopause causes urethral caruncle , burning micturition without infection and stress incontinence due to poor vascularity around the internal urinary sphincter . These symptoms are clubbed together under the term urethral syndhrome. 7)Endocrine system:- Slight virilisation as seen in the increased hirsutism is probably adrenal in origin . What Are the Non-Surgical Skin Tightening Methods to Prevent Laxity?

There are many on the market and include devices such as Refirme, TITAN, or Thermage. Titan uses infrared light (IR) to tighten the skin, Thermage uses unipolar radiofrequency energy (RF), and Refirme uses IR and bipolar RF. The ReFirme or Polaris-Laser Lift is one of the most popular FDA-approved nonablative procedures for maintaining a youthful appearance by skin tightening. Unlike Thermage, it uses a laser combined with radio-frequency energy and pulsed light. The last method of skin tightening involves laser skin resurfacing. The older methods include the non-fractionalised erbium or CO2 lasers. The newer safer methods include erbium with profractional technology, the Fraxel or the gold standard Active Fx fractionalised CO2 laser. Many doctors, including myself believe a combination of treatments (surgical and nonsurgical) get the best tightening results. The Refirme (used to be called the Polaris) is probably my favourite method as it is not especially painful, has a combination rejuvenation laser, does not cause later skin damage and has a lot of experience in the cosmetic market.
Why do you like the Polaris-Laser Lift over other RF devices?
The box says “this advanced system incorporates three forms of energy to improve skin texture and skin colour”, but I technically like the Polaris RF because it uses bipolar radiofrequency, which has been shown to be safer and more effective than monopolar. Unlike less controllable and less safe monopolar, bipolar RF does not travel through the body and this means each treatment can be customized through the system’s computer to control the depth and degree of the heating process; hence the technology can focus energy to a specific depth of skin for more accurate treatment. Newer tripolar devices may even prove to be better again. I also like it as the RF can cause the collagen to contract and tighten over a period of time and the combination diode laser can help to reduce other problems, such as redness and brown pigmentation blotches. The procedure is also rapid and bloodless. It is usual to have four sessions of treatment are performed separated by three weeks. How does the Polaris RF system actually work?
Nobody really knows how this system works but we assume radio-frequency (RF hereinafter) passes electrical energy through the tissue and resistance produces heat within the deeper dermis. This bundle of thermal energy can be accurately controlled by computer evenly to three-dimensional volumes of collagen tissue at controllable depths. This energy causes immediate collagen denaturation of the triple-helix, which is thought to be the mechanism for immediate tissue contraction. What are the benefits of Polaris-Laser Lift over other procedures and surgeries?
For a start, it is totally non-invasive. This means there are no incisions and no evidence to date of later tissue damage. There is no bleeding or bruising and the risk of scarring is minimal. A level of control can be achieved with the Polaris that is difficult with other treatments. It utilizes three different forms of energy to reach multiple skin problems. This means we can see an improvement in wrinkles, sagging and colour. There may be areas of slight swelling or redness that quickly resolves over a few days. Make-up may be applied immediately. You may resume all normal activities immediately. The full effect of your treatment develops over a few months following the procedure and can be expected to last for several months or years. As with any cosmetic procedure, the results can be expected to fade gradually over time, at which time the procedure can be repeated if desired.
Is it painful?
A Polaris-Laser Lift can be performed with topical anaesthetic cream applied about an hour before your treatment. Some patients do not require anaesthetic cream due to the cooling created by the stream of cool air that hits the skin during treatment. When the machine is pulsed, patients feel brief, heat in the treatment site followed by the immediate cooling effect of the air spray. For those patients who are sensitive, the topical anaesthetic cream is recommended.
What kind of results can I expect?
As with all RF devices, this is difficult to estimate. Some patients see results immediately. However, for most patients the results are gradual over a three to six month period as the skin tightens and more new collagen is gradually formed. Depending on the area treated, the skin laxity and your desired results, multiple treatments may be required. Skin will continue to age as it has in the past and eventually you may wish to have another anti-aging procedure. Most patients will see fine lines and wrinkles around the eyes and mouth fill in, while the deeper frown lines and grooves are softened. Facial skin and the skin along the neck can be tightened providing a mild, non-surgical browlift or facelift effect. This tightening effect can be used to firm up loose or sagging tissue in other parts of the body. This same deep Polaris tissue heating may also have benefits for treating acne, acne scarring and stretch marks. Redness and brown blotchiness also gradually improve two to three months after the treatments are complete. The worldwide safety record of this procedure has revealed very few adverse effects thus far.
How much does it cost?

The Silk Road is China’s Most Exciting Travel Destination

The number of travel agents that offer tours and services has increased significantly in recent times. China is modernising fast and it won’t be long before the original Silk Road experiences will be gone forever. Travel in China, Uzbekistan and the Middle East is safe as long as you keep away from the politics; it is affordable and provides a reasonable degree of comfort. Anyone who has travelled the Silk Road will tell you that was an unforgettable experience.
So why is the Silk Road so popular? Tours are offered by most of the major travel agents and tour companies. New itineraries have been added for 2010.
The Silk Road historically extends from Xian in central China to either the Middle East or Europe. The ancient trade routes still exist just like they did in the time of Marco Polo. There are many routes, some to Moscow in the north and south into India and Pakistan. Others rely upon international air connections.
Most tours start in Beijing. The highlights, the Imperial City, the Great Wall of China and the many places of historic interest will justify a 3-5 day stay. Add a little shopping and time to experience northern Chinese cuisine and you will be ready for your Silk Road experience.
Many international airlines fly into Beijing, Shanghai and Hong Kong. There is a lesser choice of flights to and from western China.
China has extensive internal rail connections and lines north to Mongolia, Hong Kong, Tibet and west to Moscow. For the more adventurous, there are rail links to Vietnam.
Bus access from/to Pakistan has been available along the Karakorum Highway, apart from November through April, when it is closed. However, a huge landslide in January 2010 has temporarily closed the highway. This route does have dramatic scenery but delays and discomfort are common, so be prepared. Travel in Pakistan warrants serious consideration. With the rise of the Taliban the risk for westerners has increased dramatically and independent travel is not really recommended.
Visas are required for all access points to China. Obtained these well in advance.
Train travel is popular within China, however, it does also have an extensive coach/bus network. Of course you could fly but that would really offset the main benefit of visiting China. Train travel is reliable, fast and cheap. “Soft” sleeping compartments either for four or on some routes for two persons are available or if you want to join the locals try the “hard” class. Seek assistance buying the rail tickets as few station staff speak English. The timetables can be complex. Ask a travel agency with China connections to help.
International companies including: GAP, Peregrine, World Expeditions, Travel Indo-China offer tours along the Silk Road. Most of these use a combination of coach and rail travel. Find these using Google. Some tours include a tour leader and guide. Standards of accommodation and comfort are reflected in the pricing.
Another option is to hire a guide through Chinese companies like Xinjiang Silk Road Adventures in Urumqi. Local guides can be provided on a per day basis or overall for a tour, at very reasonable prices. Tour guides are required to be licensed in China.
China’s climate varies considerably. The summers can be hot and sticky and the winters extremely cold so the best months are in Spring and Autumn. The Silk Road journey should not be rushed so allow a minimum of 14 days plus the time in Beijing. Add another ten days if you are including Uzbekistan.
The major attractions along the Silk Road are:
• Xian and the Terracotta Army
• The Labrang Monastery in Xiahe
• The Fort and Great Wall of China Museum at Jiayuguan
• Dunhuang in the vast sand dunes where you can ride the two humped Bactrian camels. Nearby are the Buddhist Mogao Caves set into a desert backdrop of the Flaming Mountains.
• Two hours from Urumqi is the spectacular lake district of China, the Heavenly Lake. Here you’ll find Kazakh people living in yurts and grazing their herds of horses, sheep and goats.
• The desert oasis of Turpan is famous for its grapes and fruits. Nearby are the ancient cities of Gaochang and Jiaohe, the Bezeklik thousand Buddha Tombs. Here you can visit the underground water systems called karez that brings water from the distant Tian Shan mountains.
• Kashgar, has been a trade route city for thousands of years. Visit the old city before it’s demolished and attend the famous Sunday animal market which although dusty is a great spectacle.
• A short train journey or flight will take you across the western Chinese border and then on to Tashkent the capital of Uzbekistan. Here the real gems of the Silk Road are to be found in the ancient cities of Samarkand, Bukhara and Khiva.
If you are looking for a vacation with a difference and you are a little adventurous, then travelling the Silk Road should be on the top of your list. It will have its challenges but what a story you can tell when you get home, not to mention your fantastic digital photographs of the highlights of this scenic journey.
